OHIO — Ohio’s first top-prize winner in the MILLIONAIRE FOR LIFE game has officially claimed the jackpot, according to the Ohio Lottery.
The winning ticket was claimed by The Angel Trust, represented by attorney Alan S. Levine.
The ticket matched all five numbers and the Millionaire Ball in the April 27 drawing. The winning numbers were 4, 15, 19, 21 and 31, with a Millionaire Ball of 4.
The jackpot offered an annuitized prize of $1 million per year for life. The trust chose the one-time cash option of $10 million instead. After mandatory federal and state taxes totaling 26.75%, the trust will receive $7.325 million.
Lottery officials said the winning ticket was purchased at Giant Eagle #1263, located at 15325 Edgecliff Ave. in Cleveland.
The prize marks the first jackpot awarded since the multi-state MILLIONAIRE FOR LIFE game launched in February. The $5 draw game gives players a chance to win either $1 million a year for life or $100,000 a year for life, with cash options available for both prizes.
Since 1974, the Ohio Lottery says it has contributed more than $34 billion to education across the state.
