
OHIO — An Ohio Lottery player who purchased a $5 ticket has claimed the first jackpot awarded through the new multi-state Millionaire for Life game.
The Ohio Lottery announced that the winner, who chose to remain anonymous, recently came forward to claim the prize through a trust. In April, the player became the first person to win the game’s top prize by matching all five numbers and the Millionaire Ball, beating odds of 1 in 22.9 million.
The winning numbers were 4, 15, 19, 21 and 31, with a Millionaire Ball of 4. The ticket was purchased at a Giant Eagle on Edgecliff Avenue in Cleveland.
The jackpot offers winners a choice between an annuity of $1 million per year for life or a one-time cash payment of $18 million. The winner elected the lump-sum option and claimed the prize under the name “The Angel Trust,” represented by attorney Alan Levine.
After mandatory federal and state tax withholdings totaling 26.75%, the trust will receive $13,185,000.
Millionaire for Life replaced the multi-state daily draw game Lucky for Life in February.
The game features nine prize tiers, with lower-level prizes awarded for matching combinations such as one white ball and the Millionaire Ball or two white balls. Players select five numbers from 1 to 58 and one Millionaire Ball number from 1 to 5.
According to the Ohio Lottery, the overall odds of winning any prize in Millionaire for Life are 1 in 8.46. Drawings are held daily at 11:15 p.m.
