OHIO — Two people were killed in a four-vehicle crash on State Route 115 near US 30 on Wednesday morning, September 10, 2025. The Ohio State Highway Patrol reported the crash occurred at approximately 8:33 a.m.
According to authorities, Sean Triplett, 40, of Florence, Kentucky, was driving a 2009 Toyota Avalon northbound when he crossed the center line and struck a southbound 2025 Chevrolet Equinox operated by Morgan Krouse, 24, of Kalida.
Triplett then continued into the southbound lane and collided with another Chevrolet Equinox, a 2022 model driven by Joann Kahle, 60, of Fort Jennings, forcing her vehicle off the road.
The initial impact caused Krouse’s vehicle to be pushed into a northbound 2008 Chevrolet Tahoe operated by Thora George, 30, of Lima. Both Triplett and Kahle were pronounced dead at the scene. Authorities noted Triplett was not wearing a seat belt. Krouse and George were treated at the scene for possible injuries and released.
The crash was investigated by the Ohio State Highway Patrol with assistance from the Allen County Coroner’s Office, American Township Fire and EMS, American Township Police, Lima Fire Department, and several towing and recovery services.
