The crash occurred around 6:30 a.m. on Feb. 7 near Jamestown, about 30 miles northwest of Indianapolis.
According to police, a Kia Soul from Iowa was traveling eastbound on I-74 when it rear-ended a semi-truck, crossed the median, and collided head-on with a pickup truck before coming to a stop in a ditch.
The driver of the Kia, identified as 27-year-old Ashley Morgan of Waterloo, Iowa, and two children in the vehicle were pronounced dead at the scene.
Another child in the car was taken to a local hospital, though the extent of their injuries was not released.
The 33-year-old driver and a passenger in the pickup truck were also transported to local hospitals with unknown injuries.
