One person dead, two injured following head-on collision in Ohio

OHIO — The Mentor Police Department is investigating a deadly head-on crash that occurred Monday afternoon near the intersection of Heisley Road and Tyler Boulevard.

According to a press release, the crash happened shortly before 5 p.m. when a Hyundai Sonata traveling northbound on Heisley Road crossed the center line and struck another vehicle head-on.

The driver of the Sonata, identified as 30-year-old Ismael Rivera of Painesville, was pronounced dead at the scene by the Mentor Fire Department.

A passenger in Rivera’s vehicle and the driver of the other car were both transported to the hospital with serious injuries.

The crash remains under investigation.
